Free nursing care plans are available online that are both comprehensive and customizable. If you are unfamiliar with nursing care plans or as a lay person would like some ideas on how they may be used to help in the care of a loved one, read this article.

The Nursing Care Plans online Care Plan Constructor is the best of the free nursing care plans we have yet seen. It is offered by publishing giant Elsevier’s health division, and is based on the 5th edition of their book, Nursing Care Plans, Nursing Diagnosis and Intervention. That medical text offers over 500 diagnoses for which nursing care plans are described.

free nursing care plans Each is based on criteria established by NANDA, the North American Nursing Diagnosis Association.

The web version has 52 diagnoses from which to choose, but you may also customize a plan or create your own if you wish. Of course, these plans are not a substitute for medical consultation or advice.

The benefit here is that many of these care plans are for conditions which afflict the majority of aging people, and can easily be understood by lay persons.

An example is the care plan for “Interrupted Family Processes. Some of the listed defining characteristics are:

  • Inability to accept or receive needed help
  • Rigidity in roles, behavior, and beliefs
  • Ineffective family decision-making process
  • These three characteristics alone are likely present in the majority of families in which aging persons are reluctant to face or discuss whether they should downsize or accept in-home assistance.

The plan on this topic provides everyone involved with a professionally crafted framework for discussion and action.

No matter what the diagnosis or technical level required in a nursing care plan, it is helpful for the health care provider, the patient and the patient's family to understand the interventions needed and the goals to be reached.

Sharing the nursing care plan among all the stake holders in this process will go a long way to smoothing the inevitable bumps in the long process of aging transitions.
